It is with great sorrow that I must announce to you the closure of Three Streams Family Health Center.  As you know we have cared for many un-insured people over the years charging rates far below normal.  Unfortunately, that is no longer sustainable and Three Streams does not have the funding to continue.

We are out of business.

For established patients we will have your medical records available for transfer to the practice of your choice, and will accommodate your immediate medical needs and prescription refills for the next 30 days.  Please use this time to get established with another office.   We have a list posted with suggestions for the other practices taking patients.
