Nutrition Program
Three Streams is pleased to have Annette Sheffield RD, a certified Diabetes Educator and Master's level nutritionist on staff providing Nutrition Services. Patients may have individual consultations with Annette, or group Diabetes Classes are offered weekly. Contact us for times and dates. Annette provided a periodic newsletter for our SmartCare Patients which is available here too! See navigation bar. It is always filled with tips for Diabetics and delicious recipes.
Annette also provides pastoral care for Three Streams and is very active in research including the SmartCare program and the PILOT STUDY: Psychometric Development of the Nutrition Quality of Life Survey with Northeastern University which Three Streams is participating in.
