Medication Assistence
Due to the overwhelming demand for our patients who need medication assistance (MAP), Three Streams is here to offer assistance. Please review all information given to you by our office. Failure to do so may cause a delay in helping you.
STEP 1- Gather all financial information needed and list of current medications you are taking.
STEP 2- Bring back to our office with all documentation. Make sure all documentation and forms are complete. Incomplete applications will not be accepted.
STEP 3- Applications will be processed and typically take 4-6 weeks once submitted. We will call you when your medications arrive at our office, unless they are direct shipped to you. All fees must be paid at our office.
