PRIMARY MEDICAL CARE
Three Streams provides primary medical care to all persons whether insured or uninsured. The most cost-effective medical care, primary care focuses on prevention and healthy behaviors.
THREE STREAMS OFFERS:
• Physical exams and ongoing care for chronic conditions
• Health screenings
• Blood testing and collection,
• Pap testing and occult blood testing onsite.
• Nutrition counseling
Our practice accepts Medicare, Medicard, Tricare, United Healthcare, Primary Physician Care, Crescent, BCBS, and most major insurances. Please bring your card and co-pay at time of service. For our un-insured patients we work on a sliding scale that is based on total family income and household size.
Financial verification is required at time of service and we ask that you bring two forms of financial income,
i.e. tax forms,bank statements, paycheck stubs, etc.
Payment is expected at time of service and we do accept Cash, Check, Visa, Master Card and Discover.
